GUIDELINES FOR HEALTH RELATED ABSENCES

  • Children too ill to participate in normal school activities should stay home whether or not the child has a fever

  • Fever of 100.5°F or more, taken orally (or 101°F taken by ear, rectally or temporal scan). Students should be fever free for 24 hours

  • Final decision and exceptions regarding attendance for reasons of health of any student will be at the discretion of the school nurse

MEDICATIONS

  • Medications taken at school require a Physician's Recommendations for Medication form, completed by the physician and signed by the parent. This form must be on file in the school office. A new form must be completed when there is a change in the type and/or dosage of medication.

HOW TO SEND MEDICATION TO SCHOOL

The medication container must be labeled and contain the following information:

An adult must bring the medication to school. DO NOT send medication with the student.

  • Child's name

  • Name of medication

  • Dosage to be given

  • Time to be given

  • For prescription medication:

  • Physician's name/phone number

  • Pharmacy name

  • Prescription number

If your child rides the bus, the medication must be given to the bus driver. Put the medication container in an envelope and seal it. Mark the outside of the envelope "To the Nurse" or "Medication."

Student Insurance

SDUSD does not carry medical or dental insurance for students injured on school grounds, while under school jurisdiction, or while participating in school district activities. However, the district does provide access to optional accident insurance. Student insurance booklets are available in the office upon parent request.
